Before we begin services, we ask you call your child’s doctor and request an occupational therapy evaluation that allows us to “evaluate and treat” your child.
Your child’s doctor can fax the OT referral to 208-441-2641.
Once we receive the referral, we will get your child scheduled for an OT evaluation at a time that is convenient for you.
